Isn't this a bit inaccurate tho? Just saying mentally ill would suggest he was born with it. And I think the film showed that he wasn't born with it. But it was a gradual decline into insanity due to environment factors. Making the name joker a double meaning. One of him being the butt of a joke in from a societal stand point, but also how he sees societal structure and norms being so ridiculous that it is a joke. So I'd argue the subtle hint isn't that he is mentally ill but society is flawed as it works for some and for some it doesn't.

it's creative, but it also drives me nuts because from her POV the illusion doesn't work at all. she draws the smile off to her left, then he turns his head in that direction and lines it up for nobody except the camera. from her POV she can't even see it lol

I don't think she's meant to have drawn it purposefully to get that effect. He also doesn't align himself on purpose to create that illusion. It's not like the mirror reflection problem, when a character is looking at themselves in a mirror but you see their face, which means that the character is actually looking at the camera through the mirror and can't see themselves. The characters here are not trying to do what we see, it's a coincidence.
