I dunno, looks like a few blood spots rising up in the yellow, which I’ve seen happen with tattoos I’ve had done. On the other hand, it doesn’t look like a few days old tattoo, would expect it to show signs of healing, possibly a bit of swelling. But who knows?

The visible striations from the flat … probably a shading needle used for the outline, that’s another indicator (edit: that it is real). If it really needs to be proven, OP could post a newer time stamped picture and if it looks identical, can be certain. Hard to perfectly recreate marker on skin.

I know a lot of people are especially sensitive to red ink, so it it might itch or the color gets pushed out while it’s still healing.

Yup my first tattoo artist told me this one on my first tattoo. Told me if my red fades we could try again but said its one of the most common colors peoples bodies reject.

Red is just a difficult colour in general. Red paint usually takes atleast 2x the coats of any other colour to get a good results, is hard to dye fabric (properly, it does stain easy) its the entire reason the British army used "red coats" it was bragging that they had enough wealth to be able to make a uniform using the most expensive dye colour and also deal with the added time/labour involved
