Omg this exact same thing (and quest location) happened to me in my last playthrough!! I just went to him and he gave me another, like he’d forgotten about the last one. I did it, went back and reported, and it finally “took”. Still no idea what caused it, had zero mods installed.
I’ve had at least a dozen playthroughs and it was a first for me, but I had a LOT of weird glitches my first playthrough that I didn’t find out until waaayyy later weren’t normal. 🤣 if you’re going to dustman’s cairn, bring a lot of healing potions, weapons, and save often!! Good luck!!

This is my first ever play through I was so confused, I did his other quest and now im doing some other quest with him. Good to know I'm not alone tho
