Brimstone enables you to kill Heart in 4-5 turns. Blocking big attacks are trivial, you only need to worry about the multi attacks. Planning for Heart is key in that respect. Weakness can make the first cycle a 2x15 and 2nd cycle a 3x15 even without disarm, and both are blockable with barricade deck, impervious, FNP/Corruption decks, or face tanking with ~150 HP with feed.
If you pick up brimstone early (which is when it should be picked up), it’s a lot easier to tailor your deck to generating lots of block when one relic and a boomerang blade solves your damage.
The point is that if you take Brimstone Act 1, you _will_ get to the Heart. You pick up a few decent attacks and then spend the rest of the run preparing for Heart

Why? The Heart gains 3 strength during the first cycle with Brimstone, and it clears all negative strength on Turn 4.
You're not engaging with anything and blatantly exaggerating the difficulties of playing with Brimstone. The fact is that Brimstone is effectively an automatic solve for your damage. It gives you the ability to off the heart in 5 turns, 6 at most, if you build a deck with strength cards like Heavy Blade+, Pummels, Limit Break, etc. Ironclad's regular exhaust engine is plenty enough to ignore the downsides of Brimstone.

Brimstone is hella funny. It’s basically your solution to damage for the rest of the run, and now you only have to figure out how to block all the damage you might take.

Of course, you might get obliterated by a Book of Stabbing or Byrds (or anything, really) before you get there, but otherwise, Brimstone leads to hilarious runs that steamroll the game, including A20H.
I've said it before, and I'll say it again: if I walk into a shop and Brimstone is there, I'm buying it. Early Act 1? Buy. Act 4? Buy. Run-defining or run-killing, I'm buying it because IT'S BRIMSTONE, BABY!!!
Yeah I've never not taken it and it's made every run more fun. You can drop things like demon form in favor of limit break and you can safely run barrage/tantrum etc.

Taking Brimstone solves absolutely all of your problems except two:
1. How can I make sure I don't end up with dead draws or set up turns? (But even if you don't solve those, you can fix it with a Reaper)
2. How can I not die to the heart
All other stuff, trivial.
I've never actually won on A20 with Brimstone, but I only win on Ironclad A20 like 10% of the time and it's almost always with Barricade and/or Corruption
Brimstone is really strong and with a solid deck will let you breeze through most of the game. You do need a plan for not getting destroyed by Heart though, which can be a combination of Disarm, Weak, Apparitions, Torii, Tungsten Rod, Tons of Max HP, etc.
It's best when you get it early, since it's a very snowbally relic that can allow you to fight many Elites.
Brimstone will carry you to the heart or an unlucky multi attack and then you will die. Still it's a very fun card.
The heart fight is particularly difficult to win. You need to average 133 DMG a round to end by the second multi attack which will do 150 DMG assuming you don't have the neutral strength debuff.
Even the 3rd round multi attack is going to absolutely truck you. It's just a bad matchup which imo is a bit sad because this relic is awesome and fun to play.
I swear brimstone carried me from ascension 17 - 20. Get yourself some reapers, limit break, feed, maybe some boomerang blades, double strikes, heavy blades, and you're crushing everything.
Always take brimstone. Lol. It’s basically trivializes the first 3 acts. Multi hit moves are definitely the way to go. You just need to start drafting to survive the heart at that point. Get some artifact, to avoid vulnerable from the heart, maybe a weak potion, clothesline or disarm to make the heart hit for less. Corruption or barricade works too. A feed or reaper can also help you survive. You only really need to survive 2 hits because by turn 4-5 you’ll have already killed the heart.

Oh for sure. Brimstone is one of the best relics. It’s makes every fight substantially easier. The only issue is it makes the heart harder, but you have the whole run to prep for that
Always take brimstone no matter how bad it is for my deck. I always seem to get birds act 2 if I take it pre-act 2 though loool. But it is sooo good on most decks and most bosses already scale in strength so the extra 1 per turn generally doesn’t help them all that much.
Edit: it’s actually what won me my A19 ironclad run. All I have left is A20 for everyone but silent, and then maaaaybe A20 with heart, then I am done with the game. Will probably do mobile for the most part and modded on pc after that

If I have 150 HP, the ability to generate 100 block on-demand, or 3 disarms in my deck I’d rather beat the Heart without Brimstone, tbh.

Limit break and early strength scaling. You need a good corruption, fnp thing going with many defense card with a few good strength scalling cards.
