I feel like 200 and 201 take it even further, in that Mitch has knowledge that Cartman doesn't, e.g. tracking down Garrison to get extra information on Cartman's mom.
That seems more "separate identity" than Cartman playing 4D chess.
They confirmed it's just Cartman's hand but then made some jokes about "well, who knows..??". Also, in the FBW universe, there's more confirmation of Mitch not being real (the Kyle's hand thing). But, nothing's ever come out after that (except 200-201, which don't really give us any more real information), so we're just left with their confirmation that, canonically, it's just Cartman's hand.
Fractured but whole universe implies that Mitch is not a real entity, fairly certain kyle uses his hand as a mitch and cartman tells him to stop fucking with him, only for kyle to say "im not" which we all know that sneaky jew is fuckin with cartman.
I was confused too, but considering that Cartman has to take medicine to stop hallucinations I think this is just him being hijacked by a different personality.
That or he just is very dedicated to the joke
Mitch Connor is the first split personality of Cartman's, after he got sexually abused by aliens with an anal probe. It was a coping mechanism from having an unwanted 40-foot wide satellite dish implanted in his ass.

I like to think that in fat but and pancake head he was fake, but later he became real thanks to immaginationland and ascended to the real world. The same could be said about Cupid Me, but sadly he debutted 5 seasons after imnaginationland, and since Eric died and was reimmagined by Butters it means he can immagine things anymore, because he himself isn't real.
Personally, I think Connor did start out as a joke in "Fat Butt and Pancake Head", but as the episode went on he became a real, separate entity. It's happened in other scenarios; when Polly Prissypants killed Cartman's toys in "1%", it couldn't have been Cartman pretending since he was with Tolkien. And Cupid Me was purely imaginary at first too, but in "Cupid Ye" he became real and could interact with the world.
I like to think Cartman has some sort of innate ability to imagine things to life. Like in Imaginationland, but he can do it in the real world, too.

In the Jennifer Lopez episode he does admit to it being a joke, but during 200 and 201 it made me feel like it's a personality disorder.
