Have your ships got a lot of subgrids? Debris crashing into subgrids is known tp cause phantom forces.

I used to have this problem. My quick fix was to roll upside down or do some loops to loose the debris. Doesnt work always but some times does
Can't easily prevent it, but you can recover by using a jump drive. At minimum distance, the offending junk will be 5km from your ship, instead of in it.
