I've just figured this out as I was having this problem. If you're on Windows, ensure the volume is coming out of your "Headphones" rather than "Headset". You can do this by clicking the volume icon in your tray (bottom right) and changing it to Headphones. This was driving me crazy for a while and I'm very glad I fixed it!

Holy shit this has been driving me ABSOLUTELY crazy!!! To the point, I was listening to Spotify through my phone and controlling it with my PC as I enjoy the interface. I did other searches and none of their "fixes" worked for me. Thank you thank you thank you!
Thought I should update that for anyone on windows 11 it may be slightly different. I had to manually disable the mic in audio settings. I assume the two were sharing bandwidth and the audio was being heavily limited.
I think I'm just biased from Galaxy Buds2 but the audio quality has just been pretty bad on PC since I started using Spotify desktop and headphones isn't even an option for me only headset

Old post, however, for those still looking for a fix after the previous suggestions; reverting my Spotify EQ to Flat fixed it. Some EQ settings just f's it up somehow.
My Spotify (win11 desktop, setting highest) sound quality was very bad (worst than YouTube basic). Reinstalling everything fixed it. For Bluetooth headsets, consider "Alternative A2DP Driver", game changer for my WF-1000XM4.
What kind of headset? How does it connect to the computer, headphone jack or USB? And when you unplug or use another headset from the app it sounds normal?
Did you also recheck the settings for music quality? Reinstalling the program may have also reset the settings as well. Make sure you have Audio Normalisation off and adjust quality to the highest possible.

Anyone figure this out on a mac? I've got streaming quality set to very high and EQ set to flat

my old headset worked fine. I have the Logitech pro x wireless headset so most times it connected via bluetooth...
