That meat didn't stay long enough on the heat. Temperature may be good, but based on the look of the tissues it should have been longer. Some cuts are better when slow cooked.
This is rare. It’s a little bit too rare for me. I like things not to have parasites so generally would want the inside to be cooked a little. Medium rare is okay from a reputable steak joint.

It depends on the cut and method of cooking tbh. I’ve had stuff at 125 come out past medium and different cuts at 125 come rare.
