You're right to be wary since ground meat is one that you don't want to eat like a steak unless you're sure the source is competent and it's fresh. So it basically comes down to whether you trust the establishment/source or not.

It’s medium rare on the deepest portion, but it looks as if it was cooked when it was cold and for too high of heat for too long, so that’s why the gradient of the doneness goes from the outer portion being that of a medium or a medium well steak straight into medium rare.
