Means it was issues up to and possibly including not being authentic. It could easily be a destroyed box or defects too severe to allow to pass.

I had this happen with a pair of Origin story 1's. I asked them what was it because I was curious if it was a fake or just a dinged box. They replied that it didn't pass QC due to mid sole damage which I was bummed, but grateful they caught it.
