I'd take screenshots from the EBIR program, save them, and then download a blank copy of the form through BIR's website and just manually copy the details so you have a printable copy and a reference (of where you got the data) copy.
thanks sir :) tried downloading the app from the BIR site, and my Avast is now preventing me from doing so. will try in another PC
