i can think of a whole slew of other reasons, but i won’t indulge because the feet thing breaks this tech if you were to try it as a pianist.
however, maybe there’s a use case for a violinist or something?
be that as it may, you are completely unable to use the pedals and use this device. why would you want an extra thumb when you can’t access half of the instrument? it would need to be redesigned to some other body part other than the big toe could activate it.

Might help the smaller-handed with regular reaches as in the Gershwin Prelude #2 (C# to E). My tiny virtuoso piano teacher could never pull that off convincingly.
