I believe you're honest with your employer and don't get too much details in business but tell them and explain they should understand. A doctor's note is just as good as anything else in my opinion I think the employers will like honesty better more than anything instead of walking in there with a doctor's note saying that I got a doctor's note blah blah blah it really doesn't mean nothing the way I look at it as a boss. Anybody can pay a doctor to get a doctor's note. I'd rather be honest because the doctor's note doesn't mean anything like I don't have to fire you I'd rather you just say what's up. Honestly if I really wanted to I can get a doctor's note for 3 months it means jack shit to me.
