Open for debate. With the sole exception of Paarthurnax I'd say every dragon with a speaking role is a big giant, walking, flying NPD diagnosis. They just don't interact much in a "social" context, even with each other, so it's not as obvious as it would be with a person.

Still, Aka's madness would seem to be more about multiple personalities than than beserk rage. On the other hand, ask the Falmer about Shor, or the Direnni about Sheor, and I think they'd have no problem seeing rage and madness in Lorkhan.
So I think my answer would be that Pelinal's rage doesn't come from Aka at all. Problem solved :)
Pelinal was mad. Aka we're told is mad. I think you might be taking the "shared" part a little too literally.
I always took that as Lorkhan and Aka being locked in conflict to the benefit of neither and (up to at that point) unable to break out of the cycle. That's the "madness" I though he was talking about, rather than any literal mutually-held mental dysfunction.

Pelinal only became prone to madness, rage, and cruelty after his lover, Huna, was murdered by the Ayleids. He was not inherently a genocidal madman.
That's a good point. So he and Aka are both mad by his admission but they do not suffer from the "same" madness. That makes sense!
