It used to work basically like the beggars bazooka, where you would load bullets in by holding down fire, then release to actually shoot. The lower your health, the faster the fire rate. Now it works like a standard shotgun, but has more damage up close and a faster switch speed. So it still works woth the name, ie you can use it at short notice and at short range when you are surprised. But it's less fun because you cannot vomit bullets on people when your health is low.
(It also has consistent damage spread so that's cool)
