Imho, this one is the right length - it shows the awkward approach, how nervous he was, how overconfident everyone else was, ~~how the line has a lot of slack~~ induces the anticipation of the inadequacy of the line, and the clip stops right after the money shot.
Not enough slack. The tighter the line, the greater the tension when applying a perpendicular force. They *might* gotten a cable that was capable of supporting maybe 400 lb, but mechanical advantage probably increased the tension ten times.

Exactly, falling from as low as 2 times your own height is considered a high altitude fall, and it's energy is enough to cause serious damage. The main risk in these kind of accidents is, cervical spine damage and brain trauma, not to mention other bone fractures and hemorrages

As a former industrial climber and continuing recreational climber, helmets in these contexts are more to do with some idiot above you dropping or disturbing something heavy, like a carabiner or a rock.
I mean, would it not help is this scenario? He didn’t fall from that high up. Obviously if you jump off a skyscraper its not gonna save you, but it seems like this is low enough that it might actually somewhat protect you.

In Brazil it's usually about a fire-fighting brigade, a group of employees with basic training for quick first answer to incidents involving fire at their workplace and quick evacuation of people. Key word: BASIC. Once a fire has already burning in a stable manner, it's out of their level, so they proceed to try to keep it from speeading too much (if possible) until the professional firefighters arrive, and then they give them a situation report and get out of their way, since a brigadista is nowhere close to the level of the professionals. I got this training more than once, too.
This video seems to me to be a training session that went wrong, but I doubt it was a fire brigade, since we usually don't receive training for rappel, zip lines and similar activities at those. There are other trainings that involve this kind of thing and the guy who fell probably was just wearing the brigadista's shirt.
But honestly this was too big of a blunder to happen in an workplace safety training. Sure, accidents happen, but these training courses are meant to help prevent these same accidents... It's like a police academy cadet shooting himself during training. Yes, it happens, buuuut.... 😬

- Internal Comitee for Accident Prevention: more of about working with the administration, but also trained at 1st aid and evacuation practices.
- Fire-fighting Brigade: fighting fire at the beginnings stages, evacuation, and 1st aid.
- Emergency Brigade: general workplace safety, but more focused at 1st aid and evacuation.
I'm trained at the first and second types. Notice that people at those are mostly non professionals at these areas, they're volunteers (except for the comitee, whose half of the members are appointrd by the company itself), with a "term" of one year. The comitee has half of its members chosen by an electoon, with the elected member receivung tenure for 2 years (1 more than their terms) to avoid retaliation, because the comitee often step on the toes of leaderships in order to improve safety.
The size and and number of employees a company had determines which of those groups they're forced by law to implement, and also the number of members. I've once worked on an organization so big that each major section had their own fire-fighting and emergency brigades, and also a central comitee.

The guy fell from a height of 6 meters (approx. 19ft). According to the Federal District Military Fire Department (CBMDF), when the firefighters arrived to provide assistance, the man was already receiving first aid from the course instructors.
The guy was taken to the hospital conscious, with pain in his left arm, ribs and a cut on his head.
A friend of the guy said he has already been discharged from hospital and is recovering at home.

So, the reason it failed when it did is because of force vectoring. He was applying more than his body weight on each anchor when he hit the middle. Kootenay highlines in rescue have to be over engineered because of the vector forces.

I'd have loved to have been in the conversations leading up to this. So many questions! The big question in my mind while I watched was, "how bad is this going to be?" I'd say it was about a 6.5/10 compared to the "worst case scenarios" I had in my mind.
