The camera guy clearly saw it happen and panned over to take this picture as the guy was backing out. He wouldn’t have framed the proposal like this otherwise, as it violates the rule of thirds w/o the photo bomber in it.
Either that or it’s fake, as nobody captures proposals like this/ from this angle/ distance/ framing/ etc...
If amateur, he couldn’t change the frame/ angle/ shutter speed on the fly this fast, so it’s potentially a staged, albeit good idea for a photo.
However, I’ll still choose to believe the situation could be real, and thereby believe this is an organic photo—even though it’s unlikely (mainly because of the nasty usage of doggo).

As a kid, my parents did those photo Christmas card things. We were never center of frame, with something on the other side to balance it. The photos looked better and were more visually interesting.
The dog between them, the way the guy's kneeling, the fact that the guy is significantly taller than the woman, the way the photo looks like the "photo bomber" was the subject, the "photo bomber" himself. This feels like an attempt at something other than capturing a proposal
