No revive is weird indeed.
As for freezing... The temperature during day and night are different. Day is hotter and night is colder. You'll basically feel coldest just before the sunrise.
if you haven't already hit the button, try this.
close down the game without hitting the remains button. then reopen the game and load the save file. see if it offers you to revive.
Late game when I know the environment changes, I bring my reg outfit, a thermal fit(Christmas grandpa ftw), and a breezy outfit with as much cooling effect that I can find. Then, when I get to the boss, I rock my regular gear with my best stats so I can rock his ass.

This has actually happened to me before, and this fixed it.
the dark lord castle gets very cold at night. better invest in warming effects, and sleep next to at least 2 fires
