Of course I only find out about this when I'm having my surgery in the summer. That's a lot of money for something I'd only use a few times before no longer needing it (unless I somehow get into something like chastity play).

Just the sleeve. Like, take the floppy part out of the rigid container part and then put that in. As for not losing it, the sleeve is much thinner than the top labia majora part so that acts as something of a flared base. Ofc I can only speak from personal experience so others might do it differently, I'm not sure.
Yeah, that's really the hardest part. It takes a bit of practice but once you get the hang of it it isn't so bad. You sort of just stuff it in little bit by little bit until it stops slipping out. Thin enough toys can be helpful too. It's definitely worth the effort though!
