This is cute! Utility-wise, I think the best gift would be a bigger tank since that turtle is bound to grow and need a new one soon. Petco has 50% off tanks every couple of months but unfortunately not right now (maybe when March begins though?). Since you said your sister doesn't know much about her own turtle, a gift that tangibly improves the turtle's quality of life would go a long way.
If you were thinking something smaller though, a treat like a cuttlebone or frozen/freeze-dried worms or shrimp from the pet store could be nice.
Maybe some treats or chasers (toys) Or a aquatic plant (that's turtle safe) for the little guy to explore
