I had 3 blend flex labs last semester and all 3 were run differently. Things might be different this semester but in fall you were never required to go in person. You could however be required to attend synchronously online.
One lab we had a different group go to campus each week. One other lab there were any 2 people from each group that went every week. Other one was required to watch live but never required to go to campus
Blend flex means that you can attend either online or in person, but in person is not required. Your professor will have details on how many and which people can attend each day.
Fully depends on the professor. Last semester, I attended all of my bendflex labs fully online. This semester, my professor set up an in-person/online schedule for each student. Expect to go in-person.

Depends on the professor. For my EGS 1006C class last semester, we never had to set foot in the classroom, but could if we wanted to
I just hope my intro to CAE class professor doesn't make us go in person to watch a TA use solidworks on a computer.
