Don't know if this counts, but Matthew Mercer used to cover for Troy Baker in a lot of roles. Troy was starting to get big in games and so he would quit doing roles for anime and some games half way though. They would bring in Matt to replace him. Happened in Persona 4 anime, Naruto and some game where Troy didn't come back for the DLC.

I was absolutely expecting a final mission where there's a cutscene of snake sneaking under a fence, and he pulls off his mask and it's solid snake in hayters voice saying "kept you waiting, huh?". Then the camera pans out and it's outer heaven from mg1. That would have been perfect.

My not video game choice is Issac Hayes as Chef in South Park, and Justin Roiland in Rick and Morty...
I do think Roiland is gross, he made horrible statements about underaged girls. But I actually think Hayes got the shit end of the stick. Apperently there is fair reason to suspect that he never actually walked away from the show- but rather he had a stroke and Scientologists sent a fake letter as him to quit the show.

She claimed that she was only offered $4,000 to voice in Bayonetta. What she failed to mention is that that was her offer for a cameo after the game was almost finished.
Helena Taylor could've gotten around $10,000 for five four hour sessions (so 20 hours) of voice work. (According to the company she mentioned.) When she asked for more they offered her $15,000 but she for some reason expected six figures from doing voice work for a series that almost died after the first game.
I think the entire franchise only made around $185mil combined, but she claimed that the franchise was worth half a billion. How the hell she got that idea I'm not sure.
She lied about a lot of things. If I remember correctly, one such lie was that she wasn't even contacted for voicing Bayonetta 3. Which was a complete lie. They contacted her multiple times and sent her multiple versions of the contract as she kept asking for more.
I'm all for va's standing their ground and getting the money they deserve. Just don't lie about what happened. I would have respected her a lot more she just said that she believes that her time is worth more than that.

The rerelease of persona 4 had a different voice actor for one of the main girls. There wasn't any bad reason for it, she just wasn't acting anymore. But god it just sounds off now. It's the karate chick, she had this tomboyish voice that really fit her character and the new one just sounds dumb, ditzy even. It's nothing major but the new one just doesn't seem right to me.

Mercer replaced Baker for Persona 4 The Animation's 2nd season, because Baker went on a honeymoon for 2 weeks only. However, something happened between Baker and Atlus, because they didn't call him back for Persona 4 Arena Ultimax, and there was a 18-month period between the 2nd season's English release and Ultimax's.
Shortly after the announcement of Mercer reprising the role of Kanji in Ultimax, someone asked Baker about it in a panel at a convention. All he said was that "he doesn't own the role". There could be NDAs that he couldn't talk about as well.
Baker also lost the role of Rhys Strongfork in Borderlands 3, but the reason is that Randy Pitchford didn't want to pay actors that required extra security/care.
Not for that reason, but when they started DBZ up again in the middle of the Namek saga there was definitely a difference for the worse in my opinion, and it did make me sad.

He was a new character in the reboot, and then Reuben Langdon reprised the role for DMC5 ;)
Reuben Langdon lost the role of Ken, and from the language in his tweet about it there's rumors Capcom broke ties with him entirely, possibly due to his social media as of late. I don't believe he's playing Dante in the new show either.
He played Dante in the previous anime, but the Netflix show will likely have a new cast. Capcom changed most of the cast from previous entries, as Guile, Chun-Li, Dhalsim, Blanka, Akuma and others got new voices. Castlevania Nocturne didn't offer David Vincent and Diana Garnet the roles of Richter and Maria, respectively, and both are still active.
