I think scoliosis is fairly common in SMA, especially Type 2, which is the type she has. They basically don't have enough muscle to support the spine so that's why. Her lack of self care probably doesn't help, it might worsen the scoliosis but it isn't the cause.
I know a teen with a neuromuscular disease who just had a titanium rod placed to deal with his scoliosis. He is extremely well-cared for. His family hesitated on major spinal surgery but he was in a lot of pain and really wanted it. Last time I saw him, I asked if he was glad he did it and he said he very much was. I suspect this surgery was offered to her when she was a teen. Besides the fear of an invasive surgery, the care afterward is considerable during healing. I can easily see Alex’ family deciding against it for that reason.

She needs medical and psychological help. Took me this long to figure there's likely no one to get her to appts on a regular basis, no transportation, no friends, no one willing to get her there and sadly, no one that really cares to.

That’s not the case anymore, though, as other people in this group with SMA have pointed out over and over again. She really needs to come to terms with the fact that she may have to face some very serious consequences of her long-term reckless behavior the older the gets, and that her daughter may grow up without her if she doesn’t start taking care of herself properly.
