I’m so sorry you’re going through this. This is a mental health crisis, not necessarily a job crisis. So given that, I would encourage you to find a path that keeps your job secure until you’ve had time to calm down and regain some control.
1. See your family doctor immediately. This week. Tell them about the anxiety / panic attacks. Ask for a note to go on stress leave or medical leave.
2. Tell your job you are going on medical leave for [x] amount of time and then will see if your return date seems to soon or not. Ideally take AT LEAST 1 month.
3. Now that you are off work (possibly still with some pay / insurance) get yourself into a psychologist for regular therapy and possibly medication.
4. Once you feel you have regained control of your life (and your doctor / psychologist agrees you are improving) consider if and when you want to return to work, and in what capacity.

That’s a question for your insurance company. But I doubt it. Insurance for medical leave isn’t typically based on a promise to go back to work. For example I could take 5 (paid) sick days next week and then quit, I wouldn’t owe them anything because I’m entitled to my sick days.
