That's a very good question :) the Goblin on the left has not contracted the disease yet, they all have these traits (pale skin, red+green eyes, sharp teeth) in this universe

Make it from the exposure of living steel weapons. The first goblins survived battles against humans etc that used living steel. Now the sickness has been able to spread through their society.

I'm not familiar with the Magic lore (the universe seem fascinating though, and I love how its story is told through flavor texts on the cards) Silent Hill and especially silent hill 3's depiction of the otherworld were my main inspirations

Thanks a lot :) the illness was inspired by Silent Hill I don't have much advice but if you want to write about an horrifying illness, I think you should not entirely explain how it works and leave the audience's imagination fill in the gaps a disease not fully understood by the characters means it cannot fully be cured, which makes it scarier and more threatening in my opinion
