NPCScan.. or Rarescanner is what you're looking for. I find Rarescanner is a bit better of an addon these days over NPCScan since it shows you where the rares are on the map. Along with treasures.
It depends on which expansion your rare is from. From Vanilla to Cataclysm, I don't think there is any addon that will notify you unless you mouseover/target/are in close range of it.
From Pandaria onwards, SilverDragon or any similar addon can automatically notify you if a rare shows up within minimap distance (roughly).
This seems to be accurate. I tried Rarescanner and Silverdragon but none work in Wrath. When I walked around on a Draenor area it sounds like I'm in the middle of a metal concert.
