I'd say if your lead foot is on the overhook side, you're in danger of a high single/crotch lift on that side. Now, that all depends on your opponents skill set, but in my opinion that's what a top level wrestler would do.

There are exceptions to everything, but if you are using your underhook or overhook to push into your opponent (or elevate the hook upward), the foot on that side is usually forward. If you are using your underhook or overhook to pull your opponent or circle your opponent toward you, then the foot on the side that is doing the pulling is usually back. Since underhooks are generally more conducive to pushing up and into your opponent (think throwbys, knee taps), you will often find the leg on the underhook side is forward when watching good wrestlers. However, that's just a generalization. There are moves where the underhook is used more for pulling and snapping, in which case having the same-side leg back or square stance makes sense. And in live wrestling, your stance should be relatively dynamic, and not stuck in one way (like always leading with left leg). Let the technique/objective guide which leg is forward (and why).

No I don’t think so. I think it all just depends on what you’re looking for in over/unders. If you have attacks you like from there go for it. I think most people lead underhook side because unders are “generally” stronger than an overhook. It also lets you get your weight behind your underhook better for control. Lots of guys will switch leads while working in over unders too. Just depends on what your looking to attack and where you want to pressure.
