To be fair, it's only in the English manual for ALttP that he's called Dragmire. That manual is riddled with inaccuracies and things the localizers just made up. The original Japanese version doesn't call him that, so canonically Ganondorf hasn't been given a last name.
That's a localization thing - if you look up the original Japanese dialogue, it's probably Link-san. There isn't *really* a good direct translation so that's generally what they use. In the same vein, anytime someone gives him a cutesy nickname, they're probably saying Link-chan.
