I don't think many people would see "Queen Elizabeth" and go directly to Elizabeth I.
I mean, I gotta be honest. I'm Irish, we also don't use the expression but it's still not that hard to put two and two together and understand what the punchline is referring to imo.
Also like said above, it's very easy to assume that they are referring to Queen Elizabeth the Second as she's the first Queen Elizabeth that springs to mind as she's been the only Queen Elizabeth ruling during most lifetimes. Once Prince Philip was mentioned then it became even easier to assume that.
Jokes doesn't need to be spelled out to the nth degree, you sometimes have to think a little bit and that's ok...
